Submit a referral, and your patient will be contacted to schedule their appointment within 2 weeks.
After every visit, you'll receive a detailed consultation report within 24 hours so your patient's care stays timely and coordinated.
We use eClinicalWorks (eCW) for our EMR. If you do also, send us a referral using the P2P direct referral system.
Don’t use eCW? No worries!
Simply send us a fax to our PHI- protected line, at 833-468-5265 or call our office directly at 208-985-6179!
Please send any patient personal health information (PHI) only through the eCW provider-to-provider channel or via the fax or phone numbers provided above, not through our email.
